Distribution Procedures & Forms

Boulder Fire & Police MPPP distributions

The various types of distribution transactions available to Boulder Fire & Police participants are as follows.

  • Lump Sum Distribution, Installments and Rollovers out of the Plan upon Separation from Service

  • In-Service Distribution and Rollovers out of the Plan (upon attaining age 50 or greater)

  • Disability (permanent disability resulting from on-duty or off-duty injury or illness)

  • Qualified Domestic Relations Order or QDRO (for divorce proceedings)

The Boulder Fire & Police MPPP is a unique Plan - the forms needed for in-service withdrawals and rollovers and separated from service withdrawals, rollovers and installment payments are unique to the Plan - you cannot use standard MissionSquare forms for almost all transactions associated with this Plan. You can access the correct forms for all transactions below. Please note that all transaction requests need to be sent to Plan Administration to obtain necessary approvals prior to submission to MissionSquare. If you do not see the form you need in the list below, please contact Plan Administration.

SEPARATED FROM SERVICE DISTRIBUTIONS

Note about taxation:

  • It is important to note that distributions taken prior to attaining age 59 ½ may be subject to an IRS early distribution penalty tax equal to 10% of the gross distribution. While exemptions to this rule exist (including divorce, early retirement, disability, and rollovers out of the Plan), participants are strongly encouraged to read the Special Tax Notice, contact the Plan Administrator, and/or consult a tax professional for guidance prior to taking a distribution from your Boulder Fire & Police account.

Your first distribution following separation from service needs to be approved by a Trustee. If you are separated from service, and have previously taken a distribution, no Trustee approval is necessary, but you still need to submit the form to Plan Administration.

All forms require your signature to be notarized.
